AdventHealth SOAR Partnership with Flagler Cares

Helping Patients Access SSI & SSDI Benefits
Many patients with serious medical or behavioral health conditions may qualify for disability benefits but find the Social Security application process overwhelming. Through AdventHealth’s partnership with Flagler Cares, eligible patients can receive individualized assistance navigating the SSI (Supplemental Security Income) and SSDI (Social Security Disability Insurance) application process.

Referral Process for the Best Outcomes

Complete the Prescreening Form to ensure that the individual is a good fit for the program. Email completed form to help@flaglercares.org or fax form to 386-463-1030 right before an individual is discharged or after discharge.
Complete a Release of Information form with the client so that Flagler Cares and AdventHealth can communicate about the client and support their success in the program. Email completed form to help@flaglercares.org or fax form to 386-463-1030.
If a Power of Attorney exists for the individual to be referred, please send a copy to help@flaglercares.org or fax form to 386-463-1030.
Complete the Referral Webform.

Flagler Cares will reach out to each referred individual within 2 business days to continue the screening process and will work with each client to determine an action plan that meets their needs. We will reach out to you if we have trouble reaching the client.

Who Should Be Referred
Please consider referring individuals who:

Are age 18 or older
Have a physical or mental condition expected to last at least 12 months or result in death
Are unable to maintain substantial employment because of their condition
Have extensive medical documentation supporting their disability
Need assistance understanding or completing the disability application process
Lack family or other support to navigate the application independently
Are able to participate in the application process, with assistance

Patients experiencing homelessness or diagnosed with a qualifying terminal illness may also be appropriate candidates for SOAR assistance.

Who Generally Should NOT Be Referred
Flagler Cares is generally unable to assist patients who:

Are currently working above Social Security’s Substantial Gainful Activity (SGA) limits
Are under age 18
Are not a US Citizen or legal resident
Have retained an attorney for their disability case
Do not have sufficient medical documentation to support a disability determination
Are unable or unwilling to communicate with Flagler Cares and participate in the application process

These situations may still warrant discussion with Flagler Cares if there are unusual circumstances.

Understanding SSI and SSDI
Although both programs provide financial assistance for individuals with disabilities, they serve different populations.

Supplemental Security Income (SSI)
SSI is a needs-based program for individuals who are aged, blind, or disabled and have very limited income and resources. Individuals do not need a work history to qualify if they otherwise meet Social Security requirements. Eligible recipients typically receive Medicaid coverage.

Social Security Disability Insurance (SSDI)
SSDI provides benefits to individuals who have worked and paid Social Security taxes but are no longer able to work because of a disabling medical condition expected to last at least 12 months or result in death. Eligible recipients generally become eligible for Medicare after the required waiting period.

Some individuals may qualify for both SSI and SSDI.
