Behavioral Health Therapist
Location: Daytona Beach, Florida

Flagler Cares, doing business as Volusia Cares, is seeking a compassionate and skilled full-time Behavioral Health Therapist to provide outpatient counseling and care coordination for adults experiencing mental health and substance use disorders. Many of the individuals we serve are involved with multiple systems—including criminal justice, child welfare, healthcare, and homelessness services—and have historically experienced significant barriers to accessing behavioral health care.
This position is ideal for someone who enjoys building meaningful therapeutic relationships while collaborating with community partners to address the complex factors affecting a person’s recovery and wellbeing.
Most Essential Work:
- Provide one-on-one care coordination and behavioral health navigation to individuals facing complex needs.
- Individual Counseling.
- Develop and monitor individualized stabilization plans addressing behavioral health, substance use, housing, employment, and other social care needs.
- Engage with clients regularly to assess progress and remove barriers to care.
What will you be doing?
- Provide individual outpatient counseling to adults experiencing mental health and substance use disorders.
- Conduct comprehensive behavioral health assessments, including biopsychosocial evaluations and risk assessments.
- Develop individualized treatment plans in partnership with clients that reflect their personal goals and strengths.
- Utilize evidence-based therapeutic approaches such as Motivational Interviewing, Cognitive Behavioral Therapy (CBT), Solution-Focused Brief Therapy, Acceptance and Commitment Therapy (ACT), and trauma-informed care.
- Support individuals throughout their recovery journey by helping them build coping skills, improve emotional wellness, reduce substance use, and achieve greater stability.
- Coordinate care with healthcare providers, recovery programs, hospitals, law enforcement, probation, child welfare, homeless service providers, and other community partners.
- Contribute to a team culture that values compassion, innovation, accountability, and whole-person care.
What we are looking for:
- Master’s degree in counseling, Social Work or a related field
- Licensed Mental Health Counselor (LMHC), Licensed Marriage & Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Florida Registered Mental Health Counseling Intern
- Knowledge of and experience using evidence-based practices such as Motivational Interviewing, Solution-Focused Therapy, and Cognitive Behavioral Therapy
- Knowledge of co-occurring mental health and substance use disorders.
- Understanding of trauma-informed, recovery-oriented, and person-centered care.
- Strong interpersonal and communication skills

Care Coordinator
Location: Palm Coast, FL

Flagler Cares is seeking a qualified candidate for a Care Coordinator serving Flagler County out of our Flagler Village office. Care Coordinators provide quality health and human services information to persons in need with the intent to link those persons to appropriate services and resources.
This role will provide outreach to potential clients and referral organizations and provide one-on-one assistance to clients through coordination, support, and education to help clients achieve health and stability. Areas of focus for care coordination may include assisting clients with benefit navigation, financial stability, behavioral health, and access to community resources. Care Coordinators also provide education, screening, and application support to individuals pursuing SSI and SSDI benefits through the Social Security Administration, helping clients understand the process, gather required documentation, and navigate available resources.
A key component of this position is supporting participants through Flagler Cares’ Partnered for Success Program. Through this program, Care Coordinators work alongside individuals and families to build a path toward economic stability and long-term success. Participants partner with a Care Coordinator to identify goals, develop an action plan, overcome barriers, and connect to services that support financial and health stability. This approach is built on shared accountability, combining ongoing encouragement, regular follow-up, and meaningful client engagement to help participants make lasting progress toward their goals.

What will you be doing?
- Assessing an individual’s needs to develop an individualized care plan to meet their goals which may include financial, housing, behavioral health, physical health, educational, vocational, and life skills goals.
- Meet with clients regularly to review progress and plan next steps.
- Provide financial education and coaching to support client’s care plan goals.
- Make referrals to community programs and assists clients in obtaining resources and services and as needed to address barriers to addressing identified needs.
- Document all interactions with clients utilizing appropriate reporting mechanisms, including our referral information system, LINC.
- Attend staff meeting, supervision, and trainings as required.
- Attend a community meeting and/or event once per month.
- Implements Motivational Interviewing and practices those skills and techniques with clients.
- Fostering a supportive and non-judgmental environment that promotes autonomy, personal growth, and resilience.
- Documenting encounters and services provided in our LINC system.
- Advocating for the needs and rights of individuals
Are you our next Care Coordinator?
- Bachelor’s degree in a human services field is highly desirable but significant experience can be considered as a substitute along with a High School Diploma.
- Experience with the SSI/SSDI benefit application process a plus.
- Gain SSI/SSDI Outreach, Access and Recovery (SOAR) certification within first six months of employment.
- Five plus years’ experience in social services navigation.
- Ability to establish rapport and build trust.
- A strong desire and willingness to participate in ongoing training and professional development opportunities for career growth.
- Commitment to upholding professional and ethical standards.
- Successfully pass a Level 2 Background Screening.
- Current Florida Driver’s License.

Job Description: Flagler Cares is looking for a compassionate and highly organized Intake & Eligibility Coordinator to serve in a multifaceted role in supporting the mission of Flagler Cares and Volusia Cares to provide accessible, compassionate, and coordinated care to community members in need. This position combines two primary areas of responsibility, intake coordination and eligibility assistance. Each is essential to ensure that individuals and families receive timely, effective support and connections to appropriate services.

- Excellent communication skills with the ability to manage multiple tasks simultaneously, prioritize competing demands, and work with a strong sense of urgency in a fast-paced environment.
