Care Coordinator

Flagler Cares is seeking two qualified candidates for Care Coordinators serving Flagler County out of our Flagler Village office. Care Coordinators provide quality health and human services information to persons in need with the intent to link those persons to appropriate services and resources. This role will provide outreach to potential clients and referral organizations and provide one-on-one assistance to clients through coordination, support, and education to help clients achieve health and stability. Areas of focus for care coordination may include assisting clients through health care navigation and meeting other needs such as housing, financial and behavioral health.

The ideal candidate would have extensive knowledge of social services in Flagler County with a focus on care planning components with appropriate services and have a proven track record of effectively coordinating care for clients.

Superior organizational skills, attention to detail and ability to prioritize multiple tasks to meet frequent deadlines is required.

This is a full time exempt/salaried position that can offer flexibility for the right self-motivated and self-driven professional.

Education and Experience: Bachelor’s degree in a human services field is highly desirable but significant experience can be considered as a substitute along with an Associate’s degree. Social services navigation experience is desirable. The ideal candidate will have a least 5 years’ experience working in the health and/or social service sector collaborating with other agencies to assist clients.

Compensation: Salary range is $40,000 to $45,000 per year for the right candidate. Benefits include a generous PTO allowance, paid holidays,100% employer-funded Health Insurance Plan and Fidelity SIMPLE IRA plan with employer match to assist with your retirement needs.

All successful candidates must have reliable transportation, possess a valid Florida driver’s license, be insured, have an acceptable driving record and complete a State of Florida mandatory level 2 background screening prior to start of employment.     Intake Specialist

    Flagler Cares is seeking a qualified candidate for an Intake Specialist serving Flagler County out of our Flagler Village office. The Intake Specialist provides initial contact with new clients and assesses client’s needs. The Intake Specialist will become familiar with local resources and services in the area and take the lead in maintaining those resources for our Community Programs Team. This role will link client with care coordination within Flagler Cares or may provide supportive referrals to partner agencies. Areas of client needs may include housing, financial, health and behavioral health needs.

    The ideal candidate would have extensive knowledge of social services in Flagler County, be able to assist clients from a place of empathy and compassion and have the ability to assess the needs of a client.

    Superior interpersonal skills, ability to maintain calm in crisis situations and skill to gather information in order to complete initial client screenings.

    This is a full time exempt/salaried position.

    Education and Experience: Associate degree in a human services field is highly desirable. Working within social services experience is preferred. The ideal candidate will have a least 2 years’ experience working in the health and/or social service sector collaborating with other agencies to assist clients.

    LPN Care Coordinator

    Flagler Cares is seeking a qualified candidate for an LPN Care Coordinator serving Flagler County out of our Flagler Village office. The LPN Care Coordinator is responsible for the assessment and care coordination of a multi-disciplinary and multi-agency Addiction Stabilization Program serving Flagler County residents with Opiate Use Disorder (OUD) to gain access to medication assisted treatment as well as other necessary medical and health supports to promote long-term recovery.

    Responsibilities: Complete nursing assessments, Clinical Opiate Withdrawal Scale (COWS), collect, interpret, and record urine drug screens and vitals. Conduct outreach to potential Addiction Stabilization Program participants, assist with enrolling participants into the Addiction Stabilization Program and follow-up with program participants. Provide education, support, and advocacy on OUD prevention and treatment. Coordinate referrals as needed and collaborate on discharge planning. Monitor delivery of services through consultation with providers and participate in case staffing and treatment team meetings.

    This is a full time exempt/salaried position.

    Education and Experience: Active Florida Nursing license is required along with CPR/BLS certification. The ideal candidate would have a minimum of one year experience in nursing. In addition, experience in care coordination is preferred. Familiarity working with participants who have mental health and/or substance use conditions is also preferred.

    Compensation: Salary range is $45,000 to $50,000 per year for the right candidate with considerations for education and experience.
