Flagler & Volusia Counties Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is a systematic effort to address public health problems in a community. The collaborative CHIP was developed with input from a variety of community partners representing both Flagler and Volusia counties. The plan defines the priorities for the community’s health.


Flagler & Volusia Counties Community Health Needs Assessment

The Community Health Needs Assessment tells the community story and provides a foundation to improve the health of the population. It is the basis for priority setting, planning, program development, policy changes, coordination of community resources, funding applications, and new ways to collaboratively use community assets to improve the health of the population.
